There are certain requirements that are placed on your doctor with respect to approved conditions. He or she will have to guarantee that they are requesting the treatment for an approved condition. Be sure that you can provide proof of residency, as that will be required when you submit your application.

In order to complete the application, you’ll need the filing fee, two recent passport-sized photos, and a letter from your doctor written within the last three months attesting to the treatment recommendation. You will also need to sign a Fingerprint Consent Form, and present yourself at the police station for fingerprinting. A copy of your photo identification and proof of residency will be requested by health unit staff, so be sure you have these documents handy. Once this is complete, the application will be reviewed by public health staff.
